Call Bighorn Medical Center for available times and dates at the various locations as well as prescription renewals. Office visits are by appointment only. Please allow 72 hours or three (3) business days for all prescriptions.

Outreach Clinics

We make it more convenient for our patients by providing some services through our outreach clinics in Thermopolis and Worland.

Scheduling for visits at our outreach clinics are by appointment only. Please call the main office in Cody to schedule an appointment.

New Patient Information

Arrive Before

  • Arrive 20 minutes prior to your scheduled appointment if you have completed the downloaded forms.
  • Arrive 35 minutes prior to your scheduled appointment if your have not completed these forms.

Bring To Your Appointment

  • Bring your current insurance cards. If you have no insurance, a minimum of $350.00 will be collected at time of appointment.
  • Your driver’s license or other photo ID to be scanned into your file in order to protect your identity.
  • Personally bring x-rays, test results and records from other physicians if pertinent to the visit. We cannot always depend on others to get this information to the office in time for your appointment.
  • A list of your medications and herbal supplements with dosages.
  • A list of your allergies and the kind of reaction you have had to each medication.
  • The completed forms that have been downloaded from this website. These are required to be completed if you have never been seen in our office before. A new copy of these forms must be completed each calendar year.
  • Co-pay for your visit as required by your insurance. We accept cash, check, MasterCard, Visa and Discover and American Express.

If Work-Related

  • Bring a copy of the first report of your injury including your claim number.
  • You will also need to provide us with your health insurance information. This will protect you if workers’ compensation contests or denies your claim. We can at that point bill your insurance company. Otherwise, the bill becomes your responsibility.
  • If workers’ compensation never contests or denies your claim for this injury, then we will not use your health insurance information. It is for your protection.

Forms

All clinic forms must be updated annually. To reduce waiting time patients can download our forms then bring them completed to your next appointment.

Disability and/or workers’ compensation forms should be filled out and signed prior to being delivered to the office. The provider portion will be completed and mailed to the appropriate entity within five business days of receipt.

Surgery Information

As a courtesy to our patients, we provide insurance preauthorization of surgeries. Patients are encouraged to contact their insurance company to verify that all requirements are met prior to surgery.

Should you have questions before surgery please call one of the following numbers. For patients of Dr Schmidt or Dr. Lee, please call (307)578-1959. For patients of Dr. Emery or Dr. Poffenbarger, please call (307)578-1991.

You will receive a pre-surgery packet that will include when and where to arrive the day of surgery as well as prescriptions for any lab work or tests which need to be completed prior to surgery, if applicable.

Please take note of specified dates within your pre-surgery packet.

Medical Records

Requests for medical records should be faxed to our office at 307-578-1918. A fee may apply. For information regarding medical record requests, please call our medical records office directly at 307-527-3343 or toll free at (866)414-1955.

Work-related injuries

Please provide a copy of the first report of your injury with your claim number. This will expedite processing related to your treatment from our office.
